Committee Payment Process
In order to be paid for your committee work, you must submit a log of your hours to the committee chair for their approval and signature. If you do not do this, you will not be paid for your work. To make this process easier for you, we are providing a Committee Work Payment Request Form in Excel format that contains all the required information and will calculate your total hours for you. When you are ready to submit, save it as a PDF file, sign (e-signature is acceptable), and email the PDF form to your committee chair for their signature. The form containing both of your signatures must be sent to your division office to start the payment process. You may use another form if you prefer; you are not required to use this specific form.
